New Medical and Photo Video Release forms are here: We are asking every youth group and every Church to submit NEW 2017 Medical and Photo/Video Release forms for every student in their group. This will cover them for the coming year (except TNT which has its own form) and get us all on the same page.
The 2017 form is required for every student on January 1st. We are shifting to a policy to update all Medical Photo Video Release forms at the beginning of the calendar year in the hopes of avoiding confusion as to who has and hasn't completed their forms. Please note that ALL FORMS ARE TO BE MAILED TO KATIE HAUN AT THE ADDRESS LISTED ON THE FORMS! Thank you!
